Are adult braces or aligners covered by insurance?
Usually no. Most dental plans limit orthodontic benefits to dependent children, so adult braces are excluded more often than covered. When a plan does include adult ortho, it works like the teen version: around 50% of the case fee, up to a separate lifetime maximum, paid in installments. Adult braces or aligners (code D8090) is the full course of treatment, billed as one case fee.
Rarely
Adult ortho is excluded on most plans
Usually excluded
A plan that says it has ortho often means ortho for kids only, and that is the norm. Read the age rule, not the category name.
50%, if at all
When adult ortho is on the plan, it typically pays about half the case fee, capped by the same lifetime maximum the kids get.
Once, ever
One lifetime ortho maximum per person on the plan. If the same carrier paid for braces years ago, some of that bucket may already be gone.
Before the consult
Aligner consults for adults are often long and free. One check tells the desk whether ortho is a covered category for this patient before the chair time is spent.
What would a real check show for this patient's D8090?
Pick a sample plan and type your fee. This is the visibility a real-time check gives the desk for every actual patient, in under a second.

When D8090 is the right code
- Patient is an adult, not a dependent teen
- Whole-mouth braces or aligners, not a few-tooth correction
- Billed as one global fee, start to finish
- Clear aligners count the same as brackets and wires
Documentation payers expect
A signed treatment plan with the total case fee, estimated months and the start date, plus records such as photos, scans or models and a panoramic X-ray. Because adult ortho is so often excluded, keep the written benefit confirmation with the chart.
Why adult braces claims get denied
What happens after the adult consult
The typical outcome is a cash conversation, so be ready for one. When the plan does pay, it pays over the case, and retention (D8680) is usually inside the fee, though a replacement retainer later is not. Adults lose coverage more easily than teens: a job change swaps the plan mid-case, and the new one may have no adult ortho at all. Knowing whether ortho is covered, at what percent, and with what waiting period before the consult keeps the payment plan honest, and that is one check.
D8090, quick answers
Does dental insurance cover braces for adults?
Usually not. Most plans limit orthodontic benefits to dependent children. Only plans that specifically include adult ortho pay on D8090, and then typically at about 50% up to a lifetime maximum.
The plan says it has ortho. Is that enough?
No. Ortho for dependents under a set age is the common design. Confirm the adult benefit specifically, and get it in writing.
Are clear aligners treated differently from braces?
Most plans treat them the same. Both bill under D8090 as full adult treatment, with the same lifetime maximum and the same exclusions.
Can an adult use the lifetime ortho max if they had braces as a teen?
Only whatever is left. If the earlier treatment was paid by the same carrier, it may count against today's lifetime maximum, depending on how the carrier tracks it. Request ortho payment history.
Is there a waiting period for adult ortho?
Often, on plans that cover it. Ortho waiting periods tend to be long, so a new enrollee may not be eligible yet. The check shows the waiting period before you schedule.
