Is laughing gas at the dentist covered by insurance?

When the plan excluded D9230, the fee shows up in full next to the filling (D2391) or cleaning (D1110) the plan did pay, and the desk gets the call. When a child's plan did cover it, the same family is surprised a year later when the age limit ends it. Each of those calls is a code check that did not happen at booking, when the plan's exclusions and age limits were one look away.
