Documentation payers expect
Payers want the actual time the patient was seen, and your posted schedule to back it up. If those evening or weekend hours are on your sign, 99051 is the code instead.
Why the after hours fee gets denied
What happens after the after hours visit
The sick visit itself carries the claim, usually 99213 or 99214, and the copay or deductible applies to that line the same as any other day. When the after hours fee is bundled, the family sees a charge on the statement they do not recognize and calls the desk. If a strep or flu test was run, that is another line. Checking plan status and the amounts left while the call is still live is what keeps that phone call from happening.

Why is there an extra charge for a night visit?
The office staffed the visit outside its normal hours. Some plans pay that fee, some fold it into the visit, and where it is not paid the practice decides whether to bill the family.
Can we refuse to pay the after hours fee?
Ask the office first. If the plan bundled it and the practice is in network, the payer agreement often bars billing you for it, and the charge should come off.
Is a weekend clinic with posted Saturday hours the same?
No. Posted weekend hours use 99051 instead, and that code has its own coverage rules with each plan.
Does Medicare pay the after hours fee?
No. Medicare bundles this code into the visit, so it is not paid separately.
Is it cheaper than urgent care?
On many copay plans a primary care visit plus this fee still costs the family less than urgent care or an emergency room trip, which is one reason some plans pay it.
