Is a vision screening at a checkup covered by insurance?
Often yes, at no cost, for young children. Most plans follow the recommendation to check vision at least once between ages 3 and 5 and cover it as preventive care. Code 99173 is the eye chart test the office runs to measure how well each eye sees, billed next to the checkup.
Ages 3 to 5
Free at those ages, often bundled
Bundled often
Plenty of plans pay the well visit and treat the eye chart as part of it, so the line shows on the claim with no separate payment.
Camera is 99177
A photo screener bills 99177 when the office reads it on site, or 99174 when a service reads it remotely. Billing the wrong one denies fast.
Not an eye exam
This is a screening, not the routine eye exam a vision plan covers. Families who need glasses go to the vision benefit, which is a separate card.
School form day
Sports and school form visits bring kids in when no checkup is due, and those are rarely preventive. Confirm the benefit before they arrive.

When 99173 is the right code
- An eye chart was used and results recorded
- Both eyes were measured with a number
- It is a screening, not a full eye exam
- Camera based screening uses 99177 instead
Documentation payers expect
Write the measured acuity for each eye in the note, not just passed. Payers reviewing this line want the numbers and the chart or method used, and the visit line usually needs modifier 25.
Why vision screening gets denied
What happens after a failed screening
A child who does not pass gets a referral to an eye doctor, and that visit runs on the medical plan or the separate vision plan depending on why they are going. Glasses come off the vision benefit with its own allowance and its own bill. Hearing screening at the same visit adds 92551. Knowing plan status, the deductible left and whether the plan wants a referral first keeps the family from finding out at the specialist's desk.
99173, quick answers
Is the eye chart test free at a checkup?
In network at a preventive visit, most plans cover it at 100% for kids, especially the screening between ages 3 and 5. Many plans simply pay it inside the checkup instead of on its own line.
Is this the same as an eye exam?
No. This is a quick screening in the pediatric office. A full exam with an eye doctor runs on the medical plan or on the separate vision plan.
Why was it not paid separately?
Plenty of plans treat it as part of the well visit payment. That is a payment rule, not a mistake on the claim.
Does Medicare pay 99173?
No. Medicare does not cover routine vision screening, so adult patients on Medicare are self pay for it.
What if it is done at a sports physical?
Sports and school form visits are usually not preventive, so the screening can land on the deductible or bundle into the visit charge. Worth telling the family up front.
