Documentation payers expect

Payers expect the age on the date of service, growth measurements, the screenings done and the counseling given. A sick problem handled the same day needs its own note and its own diagnosis.

Why well baby visits get denied

The baby is not on the policy yet, so the claim returns member not found. Verifying before the two week visit beats chasing the rejection afterward.
The sick line went out without modifier 25 on it, so the payer folded it into the checkup and paid nothing extra. It needs its own note and diagnosis.
The child turned 1 before the appointment, so 99391 denies on the age band and the claim has to go back out as 99392.
Two checkups landed closer together than the plan's edit allows, so the early catch up visit was counted and rejected.

What happens after the well baby visit

The checkup itself is free, and then the extras arrive. Vaccines given the same day bill on their own lines, a shot fee (90460) plus the vaccine. A development questionnaire (96110) at nine months and a caregiver depression screen (96161) at the earlier visits can each land outside the preventive benefit, and some plans deny 96161 because the person answering it is not the patient. Knowing the plan numbers first keeps that off the phone.

On most plans, yes. Plans that follow the preventive rules cover well baby checkups at 100% in network with no copay and no deductible, so the family pays nothing for the visit itself.

Why did we get a bill after a free checkup?

Usually because a sick problem was handled at the same visit, or a screening or lab was billed outside the preventive benefit. Both are normal and both bill separately.

Does Medicare cover 99391?

No. Medicare does not pay the preventive medicine codes, and it is almost never a baby's payer. Claims for an infant go to a parent's commercial plan, Medicaid or CHIP.
