Is nutrition or weight counseling covered by insurance?
It depends on how it is booked. Code 99401 is a short counseling visit, about 15 minutes, spent on eating, activity, weight or another risk with a patient who is not sick that day. Many plans cover it as preventive care when it is the only reason for the visit, and fold it into the checkup when both are billed on the same day.
Own day
Paid alone, bundled at the checkup
15 minutes
99401 is the short version. Thirty minutes is 99402 and pays more, so the minutes in the note decide which one holds up on review.
Same day trap
Billed alongside the annual checkup, most plans fold it into the well visit and pay nothing extra. A separate appointment is what usually gets paid.
Well patient only
These codes are for a patient with no condition being treated that day. Once a diagnosis is being managed, payers want a regular office visit instead.
Book it right
One check before scheduling shows plan status, the copay and the deductible left, so the desk books the slot knowing what the family walks into.
What would a real check show for this patient's 99401?
Pick a sample plan and type your fee. This is the visibility a real-time check gives the desk for every actual patient, in under a second.

- About 15 minutes of face to face time
- No illness is being treated that day
- Minutes and topics written in the note
Documentation payers expect
Payers want the minutes, the topics and the counseling plan agreed with the family. A preventive diagnosis, such as a body mass index percentile or a documented risk factor, has to be on the claim.
Why counseling visits get denied
What happens after the counseling visit
Most families come back for more than one session, and the second and third visits can hit a plan limit nobody mentioned at the first. Longer sessions bill as 99402. A referral to a dietitian bills as 97802 and 97803 under a different benefit with its own visit cap. The next annual checkup, 99392 or 99394, stays fully covered, but counseling billed on that same day usually folds into it. Knowing the plan numbers before the first session keeps the schedule honest.
99401, quick answers
Is this the same as the yearly checkup?
No. The checkup is its own code and covers the whole exam. This is a separate short visit spent on one topic, most often eating, activity or weight.
Why was it free last time and billed this time?
Timing. On its own day it usually pays as preventive. Billed with the checkup, most plans fold it in, and some push the balance to the family.
How many sessions will a plan cover?
It varies. Some plans cover a set number of counseling visits a year and some cover them only for certain risks, which is why the office checks before booking a series.
Does Medicare pay for 99401?
Medicare treats 99401 as non-covered and does not pay it. It uses its own obesity counseling code, G0447, with its own visit schedule and rules.
Can a nurse or a health coach do the counseling?
Payers expect a physician or another qualified provider for these codes. Who performed it has to match what the note and the claim say.
